State Seeks to Pull Agent’s License: Insurance regulators moved to revoke the license of Albert Frank Ciocatto of Irvine, owner of CSI California Specialty Services (formerly FGS Insurance Agency). Ciocatto allegedly refused to refund $150,058 in premiums to L.A. Gear after the shoemaker canceled its policies. He also allegedly failed to remit a $1-million payment from Buena Park Hyundai to Golden Eagle Insurance Co. In a separate action, Insurance Department investigators arrested James Ernest Crawford and Martin Dennis Feil, owners of an El Cajon roofing and building maintenance business, for allegedly defrauding their workers’ compensation insurer of $187,000 by misstating their workers’ occupations.
